Where resistance comes from

  • Heteroresistance — a resistant subpopulation within a susceptible-looking isolate

Selection pressure is the engine: antimicrobial exposure in humans, animals, and the environment selects for the resistant fraction that already exists. Resistance can carry a fitness cost, but compensatory mutations often erase it — which is why resistance rarely disappears when a drug is withdrawn.
